This page has many of the important forms your council, assembly or circle is expected to complete during the fraternal year.
Each unit is responsible for sending reports to the appropriate state/district officials if necessary. In every instance, copies of each report form should be kept for the council, assembly or circle files. Some forms allow officers to email the report to the Supreme Council and up to three additional people.
